Deputies say the crash occurred just before 4:30 p.m. on Thursday, Feb. 13 on Clymer Center Rd. An investigation determined a vehicle driven by 52-year-old Michael Wade of Clymer failed to keep right and struck an oncoming vehicle operated by 69-year-old David Bensink of Wattsburg, Pa.

Investigators say Bensink was not wearing a seat belt at the time of the accident. Both drivers were sent to UPMC Hamot for their injuries and Bensink died as a result of his injuries a week later on Thursday, Feb. 20.

Wade was charged with failure to keep right and no additional charges are to be filed

The Chautauqua County Sheriff’s Office says deputies were called to a report of a vehicle hitting a buggy on Quaint Road around 10:30 Sunday night. On arrival deputies learned the vehicle had left the scene. It was later located in a ditch on Stahlman Road and the operator of the vehicle was identified as 34-year-old Nicholas Taylor.

A subsequent investigation found that Taylor was driving southbound on Quaint Road when he crossed over into the opposite lane and struck a northbound Amish buggy. A 16-year-old occupant of the Amish buggy sustained minor injuries.

Taylor was arrested on multiple charges including aggravated DWI with a blood alcohol content of .18 percent or greater, leaving the scene of a personal injury accident, and unlicensed operation. He will appear in Town of Poland Court at a later date to answer to his charges.

The Chautauqua County Sheriff’s Office says several fire departments and other first responders were sent scene just after 7 p.m. Wednesday.

An Investigation found that 67-year-old Terry Telschow of Jamestown was traveling west on Route 394 when he apparently crossed the center of the road and collided head-on with an eastbound vehicle, operated by 35 -year-old Landon Armstrong on Conewango Valley.

Armstrong was transported by ambulance to UPMC Chautauqua for evaluation but did not appear to have any significant injuries.

Telschow suffered serious injuries and was flown by Mercy Flight to ECMC where he later died.

The investigation into the crash continues.

According to state police, two people inside the minivan are now listed in serious and critical conditions, while three others were also treated at a local hospital.
The crash occurred in the westbound lane of I-90 between the Dunkirk and Silver Creek exits shortly after 1 p.m. Monday.

An investigation found that Trooper – identified as 33-year-old Stephen Barker out of the Buffalo police barracks – was responding to a property damage crash when he struck the rear of the Dodge minivan driven by 75-year-old Harry Gibbons of Irving.

Two passengers in Gibbons vehicle, identified as 66-year-old George Cole of Arcade and 70-year-old Timothy McCann of Depew, were airlifted from the scene to Erie County Medical Center in Buffalo. Cole is currently listed in serious condition while McCann is in critical condition.

The westbound lanes were closed for several hours due to the crash. Police did not state whether any charges are pending.

The Chautauqua county sheriff’s office says the 17-year old-male, whose name is not being released, was driving along Lester St. in the village just after 8:30 p.m. when the vehicle left the roadway and went into the ditch. An investigation revealed the crash occurred due to an apparent medical condition.

The operator – whose name is not being released – sustained serious physical injuries and was transported to Hamot Hospital in Erie.

There are currently no charges pending in this incident.

29-year-old Jason Hallmark of Mayville is now recovering at home from a neck fracture following the crash.

Hallmark was driving a Sheriff’s Office patrol vehicle Friday on Straight Road in Sheridan around 9 a.m. when he was struck by a vehicle — operated by 36-year-old Theona Szumigala of Fredonia — traveling northbound on Center Road.

Szumigala was treated and released at Brooks hospital in Dunkirk for non-life threatening injuries.

The crash remains under investigation.

The crash took place on Route 20 just before 8 p.m. when a westbound vehicle, driven by 22-year-old Madison Dunlap of Ripley, collided with an eastbound vehicle, driven by 58-year-old Kim Robbins of Fredonia.

Dunlap was pronounced dead at the scene by Chautauqua County Coroner.

Robbins was transported to Brooks Memorial Hospital and then later to UPMC Hamot in Erie for treatment of injuries.

A portion of Route 20 was closed to traffic as a result of the crash.

The cause of the crash remains under investigation.

Chautauqua County Sheriff’s Office deputies say the crash occurred just before 6:45 p.m. Sunday on Farrington Hollow Road in Cherry Creek.

A media release says a car was backing out of a driveway on the road and was struck by a northbound motorcycle. The operator of the motorcycle was transported to Brooks Memorial Hospital where he later succumbed to his injuries. The identity of the motorcyclist has not been released at this time.

The driver of the car was identified as 47-year-old Jassen Sherman. He was cited for failure to yield the right of way when entering the roadway and is due back in the Town of Cherry Creek Court at a later date.

Chautauqua County Sheriff Deputies say 21 year-old Nicholas Hettenbaugh of Kennedy was operating a farm semi-truck carrying yard waste on Waterboro Hill Road when his engine reportedly shut off, which caused his brakes to malfunction. The vehicle then went through the intersection with Route 394, striking three other vehicles before overturning in the roadway.

No injuries were reported from the collisions. Hettenbaugh was charged with traffic related charges.

The roadway was closed for about four hours as the Chautauqua County HAZMAT and various local fire departments worked to clean up the scene.

Chautauqua County sheriff’s deputies say the crash occurred about 3:15 a.m. Sunday on Route 430.

Investigators say 22 year-old Christopher Jones of Jamestown was traveling east when he lost control of the vehicle, left the roadway, struck several trees and overturned the vehicle.

An unidentified passenger in the vehicle sustained serious injuries as a result of the accident and was sent to Hamot Medical Center for treatment of his injuries.

Jones was charged with felony vehicular assault, DWI, aggravated DWI with a blood alcohol content of .18% or great, and failure to keep right. He was arraigned in town of Sherman Court and sent to the county jail without bail.

Sheriff’s Deputies were assisted at the scene by Findley Lake Fire Department and Sherman Fire Department.
